Can anybody recommend a heat proof screed product for screeding under a fireplace?
  1.
Any fire cement will do and mix your own -
  2.
This ones supposed to be specifically for screed http://shop.vitcas.com/vitcas-heatproof-screed-17-p.asp

Is this just to level the surface of an existing hearth or build a new 250mm thick constructional hearth?

It's a levelling screed under the firebed. We've removed the old base in its entirety, replacing it with a ground-bearing 150 mm concrete slab on 250 mm EPS. We've got about another 50 mm to form a screed under the existing firebed.

Is it for an open fireplace, i.e. with firebasket or grate or similar, or just something to stand a wood stove on?

Could be either. It's actually going to be for ventilation (Ventive) but I just want to future proof it.

You could make up a concrete mix with some of these as additives, most likely fireclay and/or vermiculite

Needs to total over 250mm for an open fire/fire basket.
Less than 250mm is ok for a stove if it's certified not to raise the hearth over 100C.
